ABSTRACT

Objective:To identify core contents of health education for patients with pemphigus by using the Delphi method. Methods:On the basis of literature review and group discussions, two rounds of questionnaire survey were performed in 17 experts to evaluate the importance of health education items for patients with pemphigus by using the Delphi method. Indices including importance score, coefficient of variation, authority coefficient and Kendall coefficient of concordance were calculated to identify core contents of health education for patients with pemphigus. Results:In the two rounds of questionnaire survey, all experts complete the questionnaires, the coefficients of variation were all less than 0.35, the authority coefficient was 0.893, and the Kendall coefficients of concordance were 0.283 and 0.288 respectively ( χ2= 168.17, 181.25, respectively, both P < 0.001) . After the two rounds of expert consultation, the core contents of health education for patients with pemphigus were finally confirmed, which consisted of 6 primary items and 32 secondary items, and the primary items included "disease knowledge", "medication guidance", " dietary guidance", "skin care", "rest, activities and psychological guidance" and " follow-up and medical guidance". Conclusion:By using the Delphi method, the core contents of health education have been successfully identified for patients with pemphigus, which provide a basis for systematization and scientization of health education contents for patients with pemphigus.

ABSTRACT

Objective:To observe any improvement in hemiplegic upper limb functioning when transcranial direct current stimulation (tDCS) is combined with robot-assisted upper limb treatment, and analyze the potential mechanism of neural plasticity through diffusion tensor imaging (DTI).Methods:Twenty stroke survivors with hemiplegia were randomly divided into a treatment group and a control group, each of 10, according to a random number table. Both groups were treated with conventional medication and rehabilitation training using an upper limb robot, while the treatment group also received tDCS daily, with the current increasing from 0 to 1mA over 10 seconds, and then decreasing to 0 over twenty minutes. The experiment lasted for 15 days. The upper extremity portion of the Fugl-Meyer rating scale (UE-FMA) and the Wolf Motor Function Rating Scale (WMFT) were used to evaluate motor functioning before and after the treatment. DTI was also conducted for both groups. Results:After the treatment, the average UE-FMA and WMFT scores of the two groups were significantly higher than before the treatment, with the average UE-FMA score of the treatment group (35.32±13.25), significantly higher than that of the control group (21.80±13.93). After the treatment there were significant differences between the groups in their average FA, rFA and FAasy of the posterior limb of the internal capsule, as well as in FA and the CST length of the central anterior gyrus. Conclusion:tDCS combined with robot-assisted upper limb rehabilitation training can significantly improve the motor functioning of hemiplegic upper limbs, possibly due to neuroplasticity mechanisms that promote CST integrity and symmetry changes. tDCS can be an important adjunct therapy in clinical neurorehabilitation.

ABSTRACT

Objective:To observe and analyze the clinical effect of combining motor imagery training (MIT) with transcranial direct current stimulation (tDCS) for improving the upper limb functioning of hemiplegic stroke survivors. Methods:Ninety stroke survivors with hemiplegia were randomly divided into a conventional group (treated with tDCS) and a combination group (treated with MIT combined with tDCS), each of 45. The conventional group received 20min of tDCS using the IS200 intelligent electrical stimulator once daily, 6 times/week, for 4 weeks. The combination group received 40min of motor imagery training right after the tDCS treatment. Before and after the treatment, upper limb motor functioning was evaluated using the Fugl-Meyer assessment for the upper extremities (FMA-UE) and the Hong Kong version of a functional test for the hemiplegic upper extremity (FTHUE-HK). Surface electromyographs were recorded from the anterior deltoid and the triceps brachii muscles during maximum active shoulder flexion and elbow extension. The muscle strength of the affected limb was evaluated using the root mean square values of the integrated electromyograms (IEMGs).Results:There were no significant differences between the groups before the treatment. Afterward, significant improvement was observed in the average FMA-UE scores, FTHUE-HK scores, surface EMG indexes and iEMG values in both groups. The improvement in the combination group was significantly greater than in the conventional group. Conclusion:Combining MIT with tDCS can better improve upper limb motor functioning and muscle strength after a stroke survivors than tDCS alone.

ABSTRACT

Objective To investigate the effects of miR-335-5p on the proliferation and apoptosis of osteoblasts which were exposed to high glucose condition, and explore its possible molecular mechanisms. Methods MC3T3-E1 osteoblasts were divided into four groups:control group(5. 5 mmol/L glucose), high glucose group(HG group, 22. 0 mmol/L glucose), agomir-335-5p group(transfected with agomir-335-5p and exposed to 22. 0 mmol/L glucose) , and agomir negative control group( agomir NC group, transfected with agomir negative control and exposed to 22. 0 mmol/L glucose), cultured for 7 days. Cell proliferaton, cell apoptosis, expressions of miR-335-5p and dickkopfhomolog1(DKK1)mRNA,proteinlevelsofDKK1andcysteinylaspartate-specificproteinase-3(caspase-3) were detected using MTT, flow cytometry, quantitative realtime PCR and western blot, respectively. Results Compared with control group, the expression of miR-335-5p mRNA and cell proliferation in HG group were significantly decreased(P0. 05). The miR-335-5p mRNA expression and cell proliferation in agomir-335-5p group were higher than those in HG group and agomir NC group(P<0. 05). However, Cell apoptosis and the protein levels of DKK1 and caspase-3 in agomir-335-5p group were lower than those in HG group(P<0. 05). Conclusion High glucose inhibits the proliferation and induce the apoptosis of MC3T3-E1 osteoblast through decreasing the expression of miR-335-5p and subsequently increasing the DKK1 expression.

ABSTRACT

Aninductivelycoupledplasmamassspectrometric(ICP-MS)methodbasedonquadrupole reaction/collision ( DRC) was developed for the determination of K,Na,Ca,Mg,Al,Cu,Zn,Pb,As and Mn in food. Samples were digested with HNO3 and H2 O2 followed by dilution with ultrapure water and then analyzed directly by ICP-MS. The signal sensitivity of high concentration element ( K, Na, Ca, Mg and Al ) was suppressed easily by adjusting the voltage parameters of DRC quadrupole ( Rpa) , and the food samples could be online diluted any multiples ( i. e. from one time to million times) according to the actual need. In order to eliminate mass spectral interferences of 40 Ar35 Cl+, the operational conditions of the DRC, working with CH4 as reaction gas, were optimized for 75 As. The limits of detection for the K, Na, Ca, Mg, Al varied from 0. 013-0. 027 mg/L, and Cu, Pb, Zn, As, Mnvaried from 0. 12-0. 61 μg/L. The accurate and reliable results were obtained for validation of the ICP-MS method with food reference material according to the national standard GBW10023 , GBW10015 , GBW10018 , GBW10017 , GBW10019 and GBW10024 . As the routine method in our laboratory, the satisfactory results indicate that it has great potential for the determination of low and high concentration level elements by one single analysis in various samples.
